HIPAA Notice

1. Scope

Flexx Mortgage is a mortgage brokerage and is not a "covered entity" under the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"). However, we recognize that during the loan qualification process, borrowers may voluntarily disclose information related to their health, disability income, or medical expenses.

2. Treatment of Health-Related Information

Any health-related information shared with Flexx Mortgage will be:

• Used solely for the purpose of evaluating loan eligibility and processing your application.
• Stored using the same administrative, technical, and physical safeguards we use for all sensitive financial data.
• Disclosed only to lenders, investors, and service providers necessary to process your loan, or as required by law.
• Never sold or used for marketing purposes.

3. Your Choices

You are not required to disclose health-related information to apply for a loan. If you do choose to share such information, you may request access, correction, or deletion of that data at any time by contacting us.

4. If You Are a Healthcare Professional

If you are applying for a mortgage as a healthcare professional (e.g., physician, dentist, nurse practitioner) and your income documentation includes information about patients or protected health information ("PHI"), please redact such information before submission. Flexx Mortgage does not need, request, or want to receive PHI.
